I, _______________________, the coach for the _______team of the Players Choice Volleyball Club, have read the posted information on the Club Web Site (players-choice.com) concerning the policies and practices of the Players Choice Volleyball Club (hereafter called the club or PCVC). I understand that once registered with PCVC by signing this document, which I will be unable to transfer to any other club or coach in any USAV Regional or National Championship during the 2003 season. My coaching services will be provided for the course of one season from November 1, 2002 to May 31, 2003 and that I must reapply for this position each year. I also agree to abide by the terms and conditions outlined in this contract below;

1. Coach agrees to abide by all the rules and policies of the OVR and PCVC. Any failure to comply may be grounds for immediate termination of this agreement.
2. The club staff will review the coaching agreement. Agreements must be accompanied by a resume and/or a written summary of credentials and experience. In the event of a dispute between the club and the coach, the club and/or the coach will seek assistance from the club staff.
3. The coach agrees not to hold PCVC responsible for personal injury or disability encored while this agreement is in effect.
4. The coach agrees to operate as a good faith ambassador for the club.
5. The coach will provide organized and safe instruction and training for various skill levels, and will monitor performance for purposes of evaluating skill levels for recognition or assignment of competitive entries.
6. The coach may provide input concerning finances, competition schedules, demonstrations, special events, and training trips. However, the director will make the final decision regarding any club matter.
7. The club agrees to provide compensation to the head coach.
8. The agreement to coach becomes effective when signed by the club director.
9. The coach will be required to conduct themselves, their players and the players parents or spectators in a professional and exemplary manner at all times. Violations including official sanctions, warnings, game dismissals or disqualifications must be reviewed by the director and may be grounds for immediate dismissal.
